John Q. Archibald (Denzel Washington) is an ordinary man who works in a factory and lives happily with his wife Denise (Kimberly Elise) and son Michael (Daniel E. Smith). Until Michael becomes seriously ill, needing a heart transplant urgently to survive. Unable to pay for the operation and with his family's health insurance not covering such expenses, John Q. finds himself then struggling against time for his son's survival. In a desperate attitude, he then decides to take the entire emergency department of a hospital hostage, going on to discuss a solution for the case with a police negotiator (Robert Duvall) and an impatient police chief (Ray Liotta), who want to close the case as soon as possible.
